Your Role:
Director of Case Management As the Director of Case Management, you will be at the forefront of driving excellence in patient care and optimizing healthcare delivery.
Your responsibilities will include:
Leadership:
Manage and lead the Case Management Department, overseeing various functions including Bed Board/Clinical Intake, Disease Management, Social Services, Discharge Planning, Precertification, and Denial Management.
Inspire, guide, and mentor a talented team of professionals.
Strategic Planning:
Set strategic goals, performance standards, and operating policies to ensure the highest quality of patient care.
Interpret and implement hospital policies, regulations, and standards effectively.
Innovation:
Drive innovation in case management, discharge planning, and care coordination.
Identify opportunities for cost reduction without compromising patient care quality.
Collaboration:
Coordinate, negotiate, and procure services and resources to optimize the care of complex patients.
Foster collaboration among interdisciplinary teams to achieve efficient and effective patient outcomes.
Evaluation:
Assess the effectiveness of Case Management services, especially related to reimbursement for inpatient and outpatient care.
Use data-driven insights to continuously improve processes.
Development:
Develop clinically-based case management strategies that enhance the delivery of quality healthcare.
Ensure appropriate resource utilization across the continuum of care.
Qualifications:
Bachelor's or Master's degree in Healthcare Administration or related field.
Extensive experience in healthcare management and case management leadership.
Strong understanding of healthcare regulations, policies, and reimbursement processes.
Exceptional communication, leadership, and team-building skills.
