As the RN Director of Case Management, you will play a crucial role in managing the Case Management Department.
Your responsibilities will include overseeing Bed Board/Clinical Intake, Disease Management, Social Services, Discharge Planning, Precertification, and Denial management.
You will establish goals, objectives, and performance standards, develop operating policies and procedures, and interpret hospital policies and regulations to staff, patients, medical staff, and the public.
Key
Responsibilities:
Manage the Case Management Department to ensure the highest quality of patient care and cost-efficient outcomes.
Evaluate the effectiveness of Case Management services related to reimbursement for inpatient and outpatient services.
Coordinate and procure services and resources to manage the care of complex patients and identify opportunities to reduce costs while maintaining quality care.
Develop clinically based case management, discharge planning, and care coordination to optimize healthcare delivery.
Lead and direct the Case Management team, providing guidance and support to achieve departmental goals and objectives.
Requirements:
BSN and Master's degree in Nursing or other related fieldProfessional Registered Nursing licensure in the state of GA Minimum of five (5) years of clinical experience in Case ManagementExperience in managing/directing Discharge teams in an acute care short-term hospitalExperience in a 250
