Spring Harbor's MDS Coordinator coordinates the MDS (Minimum Data Set) process for all residents and performs necessary assessments related to the process in a timely manner in accordance with regulations.
Educates and trains appropriate staff in accurate ADL documentation as well as monitoring documentation regularly to ensure accuracy and completion.
Responsibilities:
Ensures that MDS, CAAs and other required assessments are completed timely and accurately in accordance with regulations; Assesses and monitors patients' health in accordance with medical laws, regulations and guidelines; Collects patient data and organizes information for accurate submission of MDS reports; Ensures baseline and comprehensive care plans are developed based on each individual residents' needs from data collected from health care team members per CMS requirements and guidelines as well as utilizing a person-centered care approach; Reviews medical records regularly to ensure that changes have been incorporated into residents' care plans; Coordinate and schedule Care Conferences per regulation and as needed along with Social Services; Triple Check; PDPM mapping; Provides education and training for appropriate staff upon hire, at least quarterly and as needed on ADL documentation; Monitors ADL documentation on a regular and timely basis to ensure completion and accuracy; Obtains prior authorization for insurance; Medicare certs and re-certs; Ensures ICD-10 coding is done for all medical diagnoses; Reviews QM reports monthly for accuracy and opportunities for improvement; Participates in QAPI process; Takes weekend call per rotating schedule.

Qualifications:
Must be a Registered Nurses (RNs) or a Licensed Practical Nurses (LPNs) that has received on-the-job training, or completed an MDS-training program.
Candidates must be familiar with MDS 3.
0, and have an in-depth working knowledge of the RAI user manual and RUG categories.
Attention to detail is a must.
Knowledge of current state, federal, and CMS regulations, and be able to relate professionally, positively, and cooperatively with staff, residents and family members.
